The University of California partners with Liberty Mutual in the administration of the University’s Disability Insurance Plans. The University’s disability benefits (Short-Term Disability, Supplemental Disability) create a comprehensive safety net to protect you from financial hardship over the course of your disability period, whether a few months or long term:
To file a disability claim:
1. Report your claim via www.mylibertyconnection.com.
First time users register as “new user?” You should use Company code University.
2. Attached is Liberty Mutual’s Reporting Your Disability Claim form. Sign and date the Authorization to Release Information section, and leave with your healthcare provider at your earliest convenience.
3. Attached is the Medical Leave of Absence Request form. Complete Section 1 and have your healthcare provider complete Section 2. Return the form to the UC San Diego Health System Benefits Office.
