Military Leave

Employees going on an approved military leave may be eligible to continue some of your UC sponsored benefits. For those employees returning from Military Leave, it is important to contact your Benefits Office at 619-543-7585 or at to re-enroll/reinstate your health and welfare benefits within 31 days of your return.

You may also request an adjustment of your service credit for your University of California Retirement Plan (UCRP) by completing UBEN132 “Service Credit Verification Request” available at your Benefits Office. You will be required to include your military service discharge papers and salary history card showing dates of leave.

Your Benefits representative will assist you in completing and mailing the form to UC HR/Benefits at the UC Office of the President.
