Instruct patient to defecate into
clean, dry bedpan without mineral oil. Remove portion of specimen with tongue
blades and place in collection container. Submit two or three specimens on
separate days to increase the probability of isolating a pathogen. If
antibiotics are to be initiated, then it is acceptable to submit 2 samples in a
single day, as long as the samples are from separate bowel movements. The best
preservative for enteric pathogens is modified Carey-Blair transport
(inappropriate for C. Difficile, however). Routine stool culture includes
culture for Salmonella, Shigella, Campylobacter, and Aeromonas. Specify specific
pathogens such as E.coli 0157:h7, Yersinia, Vibrio cholera, Candida (or fungus),
Group A strep, Group B strep, Staph aureus, or Clostridium difficile. If these
other pathogens are not specified on the requisition they may not be isolated.
There must also be a specific request if stool is to be examined for ova &
parasites (includes Cryptosporidium), Microsporidium, Cyclospora, or an amoeba
culture. Clinical significance of culture and general considerations: to
identify infectious organisms occurring in the lower gastrointestinal tract. |
