REQUISITION AND ORDER/ENTRY INFORMATION for BONE MARROW SERVICE

Bone Marrow Service is not a procedure which is offered on any of the current forms.
 
Bone marrow service must be scheduled by calling the laboratory at ex. 32250. Forms may vary depending upon the procedures requested.  The correct forms will be the responsibility of the Hematology Specialist.  However, the physician will be responsible to fill out the Surgical Pathology Form and have the patient consent form signed prior to the Bone Marrow procedure.
