Executive Health and Wellness: Billing and Insurance

The information below applies only to the Executive Health and Wellness program's customized health assessment.

If your services are covered under a corporate contract with our EHW program, costs and expenses will be paid by the contracting organization. Any services we provide that are not covered under such contract will be the responsibility of the individual and will be due on the date of the visit.

If you enter into a private (self-pay) contract, you agree to be responsible for the amount due.

We do not bill or collect from health insurance plans. Similarly, we do not investigate the types of services that may or may not be covered by your health insurance plan.

We will provide you with your visit schedule prior to your assessment. The balance payment will be due on the date of your services. Upon completion of your health assessment, we will provide an updated invoice.
