All of us face problems in our lives and no matter how hard we work to solve an issue, it's bound to affect those around us. It's also likely to undermine our effectiveness at work.
Recognizing this, your organization has chosen UBH's EAP to make confidential, professional counseling services available to you and your eligible family members. So when you're faced with a problem, call your EAP. Remember that the right time to seek help is as soon as possible - don't wait until the problem reaches the critical stage. We're here 24 hours a day, 365 days a year.
The EAP is a counseling and consultation service designed to help you and your eligible family members with a wide range of personal issues. The services are confidential, and the cost has been covered by your organization.
UBH can help with:
Benefit eligible employees of UC San Diego Health System, you are eligible for participation in the United Behavioral Health (UBH) Employee Assistance Program (EAP).
All household members, including your lawful spouse or domestic partner and your unmarried, dependent children are also eligible. Children are defined as natural, adopted, foster, or step-children age 23 or less, or of any age if incapable of self-sustaining employment due to a mental or physical disability. Children serving in the military are not eligible.
Call UBH directly, toll-free, using 1-866-808-6205. UBH is available any time, day or night.
You may choose to obtain your counseling sessions either through telephone consultation or by in-person appointment. Whichever you choose, your personal affairs will be discussed with a professional.
UBH will provide a referral, and an appointment can be set up promptly for a time convenient to you. Legal, elder care, child care, financial, federal tax, pre-retirement, and organizing life's affairs consultations are provided by telephone only.
A counselor from the UBH panel will be assigned to you. UBH makes every attempt to select a counselor who will meet your needs and with whom you will feel comfortable. If you are dissatisfied with your counselor, contact UBH to discuss your preference and arrange for a new counselor.
No. The full cost of the EAP service has been paid by your organization. There are no co-payments, co-insurance, or deductible payments applicable to UBH services.
All professional counselors are employed by or under contract with UBH. You will not be liable to the UBH counselor for any fees covered by EAP services under any circumstances.
If you desire additional services either not covered by this program, or provided by counselors who do not have a contract with UBH, you will be responsible for their payment.
How many sessions are available?
You and your household members are entitled to receive up to three (3) sessions per eligible person per incident for all available services through the program.
UBH will consider one of your counseling sessions used if you fail to cancel at least 24 hours in advance, unless the appointment is messed because of an emergency or circumstances beyond your control.
When does coverage begin and end?
You are eligible to receive EAP services for as long as you are an employee of UC San Diego Health System. Your eligibility ends upon termination of employment, or when your employer's contract with UBH is no longer in force. Your household members are covered during the same time you are.
Upon termination of employment, you and/or your eligible household members may continue coverage under COBRA. Your coverage cannot be cancelled, nor can you be denied renewed coverage, because of your health status or requirements for service. If you think this has happened, you may request a review by the California Department of Corporations. Your coverage can be terminated by UBH for fraud or deception in the use of counseling services.
May I continue coverage following termination of employment?
You are not eligible to receive UBH services on an individual basis following the end of employment, unless you elect coverage under COBRA.
UBH's EAP services are provided confidentially by licensed and qualified counselors. These highly training specialists have been carefully selected for their professional experience, reputations, and expertise. The UBH network extends across the country, so you'll be able to see a professional counselor close to your home or workplace.
All information is privileged in accordance with the psychotherapist - patient privilege and/or other applicable laws. Employees who use the counseling services are assured that their problem and its source, treatment, and resolution will always be afforded the maximum confidentiality permitted by law.
