Separations

All separations require a Final Payment Request form completed online. Forms must be submitted at least 72 hours prior to the separation date so that the final check is ready on separation date.

Final checks cannot be directly deposited to an employee's bank account. All final checks are sent to the Cashier's Office in Hillcrest..

Please contact Employee & Labor Relations for any "Release from Employment" prior to submitting the Final Payment Request Form.
