| All specimen containers must
be labeled with the patient's name, medical record number, patient number,

| date and initials.
Specimens not properly labeled are NOT accepted by the laboratory. |

| Labeling the specimen
container is the responsibility of the person obtaining the specimen. |
| |
| Specimens MUST be properly
identified by checking the name and medical record number on the patient's |
| wrist band against the name
and medical record number on the label. |
