Registration
To register as a new patient or to update your registration:
- If you are new to UCSD Medical Center, you must register as a new patient prior to scheduling any appointment. This is done by telephone and only takes a few minutes.
Call (888) 309-8273, Monday - Friday, 8 a.m. to 5 p.m. Select the option that says, "New Patient Registration."
- If you are an established UCSD Medical Center patient and your ID card has expired or you have a change of address/phone number/insurance/ER contact, you must update your registration.

- If you seek urgent care through the emergency room, you will be registered when you arrive. If possible, bring your insurance cards with you.
If you are admitted via emergency medical services (911) and are unable to register yourself, a patient services representative will assist you or your family in registering retroactively. When you initially arrive via ambulance and if you are unable to respond, you will be registered under a pseudonym depending on your type of illness/injury. Later, your records will be merged.
