MyChart Anticoagulation Program

The MyChart Anticoagulation Program (MAP) is an electronic monitoring program for patients of the UC San Diego Health System Anticoagulation Clinic.  MAP allows for remote monitoring of the INR and therefore less frequent visits to the Anticoagulation Clinic.  MAP is tailored to select established patients that have access to MyChart and stable anticoagulation or a home INR monitor. 

There are two MAP choices:

MAP-Lab

  • Monthly INR drawn via peripheral venipuncture at any UC San Diego laboratory
  • Communication of results and assessment of current medical status primarily via MyChart
  • Quarterly appointments at the Anticoagulation Clinic for face-to-face evaluation

MAP-Home

  • Weekly INR self-drawn at home on personal INR monitor
  • Communication of results and assessment of current medical status primarily via MyChart
  • Quarterly appointments at the Anticoagulation Clinic for face-to-face evaluation
