Post-Procedure

Assess bleeding risk/hemostasis prior to restarting warfarin.
Restart warfarin 12-24 hours post-op.
Restart LMWH or UFH 24-48 hours post-op, based on risk.
Check INR within 3-5 days of restarting warfarin.
Check platelet count if using UFH.
