Chronic Kidney Disease Clinic |

The Chronic Kidney Disease Clinic provides specialized medical services to patients with chronic kidney disease stage 3-5, with the goal of slowing the progression of chronic kidney disease and preparing patients for renal replacement therapies (dialysis) and/or transplantation. Treatment is offered by a multidisciplinary team including a dietitian, case manager, nurse, pharmacies and physician.
Clinic hours are every Monday from 1-5 pm.
