We strive for these six important C’s of primary care:
First CONTACT
Let us be the first place you turn to.
Continuity of Care
We develop long-term relationships with our patients.
Comprehensiveness
Almost all of the time, we will be able to help you directly.
Care Coordination
When needed, we coordinate referrals to specialists.
Understand the Context
It’s important we understand your health concerns within the context of your life circumstances.
Competence
We practice evidence-based medicine.
In addition to the general preventive and acute care we provide, here are some other features and services we offer:
Advanced Access
Now you can make appointments on short notice and be seen the same day or within a couple days.
This secure online tool allows you to view your personal health record, communicate with your clinic, request appointments and receive important health reminders.
Electronic Medical Records
Our electronic medical records system allows us to be “paperless.” We can electronically order prescriptions and refills, send orders directly to the lab or radiology department, and make referrals to specialists. It provides us with a way to track your care throughout our Health System so we can deliver the best and most coordinated care possible.
Obstetrical services are available in all three offices.
Our Sports Medicine physicians are available to address and treat your musculoskeletal and orthopedic problems.
For patients with certain chronic diseases and conditions, such as diabetes, we offer small group appointments to help them work toward better management of their health condition or disease.
Chronic Care Model
This new model of care involves bringing together a multidisciplinary team of health professionals to develop the best approach for treating chronic illnesses. Patients also play a more active role in managing their own chronic illnesses.
Pharmacy Medication Management
With the assistance of our Skaggs School of Pharmacy faculty, students and residents, we can now offer assistance to patients with complex medication lists who are having difficulties with side effects, costs or just being able to manage their medications regularly and safely.
Collaborative Care
Our therapists can provide therapy for common mental health concerns and can help patients make behavioral changes in order to better manage chronic health conditions.
Population Medicine
We want all of our patients to improve their health, not just those who decide to come and see us on a particular day. We will try to look out for you even when you aren’t visiting us. This means reminders for preventive services like cancer screening and immunizations, updates about current or new medications, and health information resources.
