Screening: The initial screening tool will be the Alcohol, Smoking, and Substance Involvement Screening Test (ASSIST) and/or the Michigan Alcoholism Screening Test-Geriatric version (MAST-G) for use with adults and 55 or older.
Brief Intervention Services: Services are based upon the results of the screening and focus on increasing insight and awareness regarding substance use and motivation toward behavioral change.
Referrals: Staff assures that the appropriate referrals to treatment are provided to those identified as needing more extensive treatment with access to specialty care or other resources. Some examples include referrals for:
Case Management and Brief Treatment Services: Staff provides brief intensive case management support for up to 90 days and utilizes various means to successfully resolve immediate life crisis issues or link clients to needed counseling, health, legal, or benefit resources. The Intensive Case Management Services focus on individual, family, and community strengths to both minimize risk and foster resilience.
