Each SOCARE assessment is tailored to the individual. To ensure the accuracy of the assessment, it is helpful if the patient’s family participates in the process, providing background information as needed.
SOCARE assessments involve five to six visits with a team that specializes in evaluating and caring for seniors. The team may include a neuropsychologist, geriatrician, neurologist, and social worker.
At the first visit, we perform an evaluation that typically includes:
At the end of your first appointment, you'll receive information about scheduling your other visits, which may include a visit to a neurologist or psychiatrist, detailed psychometric testing, or additional labwork or imaging
The last appointment with SOCARE is the "family conference." At this visit, we may suggest follow-up care with a neurologist, neuropsychologist, or a geriatrician. SOCARE does not arrange follow-up care, but we do provide contact information.
