Each SOCARE assessment is tailored to the individual. To ensure the accuracy of the assessment, it is helpful if the patient’s family participates in the process, providing background information as needed.
SOCARE assessments involve five to six visits with a team that specializes in the evaluation and care of seniors. The team may include a nurse practitioner, neuropsychologist, geriatrician, neurologist, and social worker.
Medical and Neurological Evaluation
Details of this evaluation depend on the memory screening test results. This visit typically includes the following:
- Medical history, including review of outside records
- Comprehensive medical, neurological and behavioral examination
- Assessment of social needs
- Review of diagnostic laboratory tests
- Discussion of all findings and recommendations for future healthy care treatment
Memory Screening
This session includes focused testing of memory and other abilities by a psychometrist. The visit lasts 45 to 60 minutes. A report is prepared and sent to the patient's referring physician.
Further Follow-up
Additional evaluation or follow-up will be arranged as needed. This may include a visit to a neurologist or psychiatrist, or detailed psychometric testing.
