Cardiovascular Risk Self-Assessment

You may be at risk for developing cardiovascular disease (CVD), including heart disease or stroke.  CVD is the single leading cause of death among American women.

Please note:  This assessment only provides a quick rating of your cardiovascular health and is not designed to take the place of seeing a physician.  For assistance or to submit the completed assessment, please call the UCSD Women’s Cardiovascular Health Program at (858) 657-8530.

Answering “Yes” to ANY question on the assessment indicates that you have an increased risk for developing cardiovascular disease.  If you answered “Yes” to MORE THAN 3 of the questions, you are at higher risk for developing cardiovascular disease, and should consider speaking with your doctor or calling UCSD for a cardiovascular health evaluation.
