Bachman Proximity Device Request Form: The Bachman parking structure has security gates at each entrance. A student must go to the Bachman Parking Office on the 2nd floor, submit the form and receive their proximity chip in order to open the gates. The Bachman Office will also be the site to purchase occasional use permits or daily permits for parking at Hillcrest. Thornton does not have gates (no proximity chip is needed); occasional use or daily permits can be purchased at the Thornton Cafeteria or Machines in the Employee Parking Lot.
Evaluations: All students must complete a brief confidential online Nursing Student Evaluation (please select correct evaluation for your rotation).

Computer Security Agreement: Please have each student and faculty complete this agreement ONLINE prior to starting assigned rotation. Information Services will assign computer access once received.
Confidentiality and Non-Disclosure Agreement: Please have each student and faculty complete agreement and submit to the Nursing School Liaison.
UC Learning Center: Access the UC Learning Center under NON-UCSD Employees. Username is the DOB (date of birth) and the last 4 numbers of the Social Security # (with no spaces) for a total of 12 digits (mm/dd/yyyy+last 4 of SS#). Students and Faculty must complete ALL courses assigned under CURRENT REGISTRATION tab. PRINT certificate of completion and submit to Nursing School Liaison.
Faculty Orientation Record: All NEW faculty must complete the Faculty Orientation Record if they have not taught at UCSD before. Please submit completed form to Nursing School Liaison prior to assigned rotation.
PYXIS Access Request: All students and faculty administering medications must complete the PYXIS Access Request form (be sure to select a password). Please add (upper right hand corner) name of faculty, school, all units for rotation and start and end dates. Submit completed forms to Nursing School Liaison prior to starting assigned rotation.
Student/Faculty Document Checklist: Faculty may use this as a tracking tool to ensure all required documents are collected for each student. Submit form with required documents to Nursing School Liaison.
Student Nurse Extern II: Externships are available for students currently enrolled in an affiliated nursing school externship program. ADN students must have completed 3 semesters (be in 4th semester by the start of externship. BSN Students must have completed 4 semesters (be in 5th or 6th semester by start of externship. Externships are an unpaid commitment for a total of 120 hours. Complete application and email with required documents to firstname.lastname@example.org. All fields must be completed for consideration. If you are not attending an affiliated school please do not apply as we cannot accept your application.
Student Orientation Day Sign-In Sheet: Faculty to submit completed sheet with student signatures to the Nursing School Liaison.
Student Orientation Record: Type and email Student Orientation Records to Nursing School Liaison. UCSD can accept Student Orientation Records emailed with SS#. For schools that do not allow SS# to be emailed faculty will email the Student Orientation Record without SS# and ALSO fax a copy with SS# to Nursing School Liaison.
