Current UC employees: You can choose between UC Blue & Gold HMO or UC Care PPO to receive care at UC San Diego Health.
- UC Blue & Gold plan: After you select this plan in UC Path, you will be asked to choose a primary care provider. Use this
list of primary care location codes to find your preferred primary care location. (You do not need to designate a certain provider.) Do not click "Select a Provider" next to the field; instead, after reviewing the checkboxes below, choose "Save and Continue."
- UC Care: This plan is administered by Anthem Blue Cross. There's no need to choose a primary care physician. Your most optimal benefits will be applied when you see a UC San Diego Health provider or other Tier One provider.
