How to Switch Your Care to UC San Diego Health

 

If you're ready to switch your care to UC San Diego Health, it's easy to do. Depending on the type of health insurance you have, you may be able to change to one of our primary care providers (PCPs) at any time.

 

1. Confirm that UC San Diego Health is in-network with your insurance plan.

 

What type of health insurance do you have?

  • Check Medicare Plans Accepted at UC San Diego Health to see if we accept your current plan. If so, you may be able to change to a UC San Diego Health primary care provider (PCP) at any time.

Medi-Cal or Medi-Connect (Medicare and Medi-Cal)

Covered California (also known as "Obamacare," Affordable Care Act or the state health exchange)

Health insurance through an employer

  • Check Insurance Plans Accepted at UC San Diego Health to see if we accept your current plan. If so, you may be able to change to a UC San Diego Health primary care provider (PCP) at any time.
  • To select a new plan through your employer, call the health plan directly to verify if UC San Diego Health is in the network. They may ask you for your employer information and our Tax ID numbers, which are Medical Group Tax ID # 23-7064656 and Medical Center Tax ID #33-05994954.

Private commercial insurance

  • Check Insurance Plans Accepted at UC San Diego Health to see if we accept your current plan. If so, you may be able to change to a UC San Diego Health primary care provider (PCP) at any time.
  • If you are selecting a new plan from the list above, call the health plan directly first to verify if UC San Diego Health is in the network. They may ask for our Tax ID numbers, which are Medical Group Tax ID # 23-7064656 and Medical Center Tax ID #33-05994954

UC employee insurance

Current UC employees: You can choose between UC Blue & Gold HMO or UC Care PPO to receive care at UC San Diego Health.

  • UC Blue & Gold plan: After you select this plan in UC Path, you will be asked to choose a primary care provider. Use this list of primary care location codes to find your preferred primary care location. (You do not need to designate a certain provider.) Do not click "Select a Provider" next to the field; instead, after reviewing the checkboxes below, choose "Save and Continue."
  • UC Care: This plan is administered by Anthem Blue Cross. There's no need to choose a primary care physician. Your most optimal benefits will be applied when you see a UC San Diego Health provider or other Tier One provider.

2. Call our Patient Navigators at 800-926-8273.

 

We'll help you to register with UC San Diego Health, choose your new doctor, and schedule any appointments. You can also make an appointment online instead.      

 

3. Sign up for MyUCSDChart.

 

MyUCSDChart securely houses your electronic medical record online, making it portable and accessible from anywhere you go.

 

4. Transfer your medical records and prescriptions to UC San Diego Health.
