Family Medicine Philosophy

UC San Diego Health is proud to provide a forward-looking, formally recognized way of practicing primary care, emphasizing a team-based approach to health.

The core of the model is the designation of your primary care team – your family doctor, the nurses and medical assistants – as your medical home.

“Your medical home becomes the hub for all your medical care,” said Tyson Ikeda, MD, a primary care physician and clinical services chief of Family Medicine at UC San Diego Health. “We coordinate all aspects of your health care, make sure you get appointments to see specialists, review any test results or notes and then discuss with you your health plan strategy. We help make sure you stay up to date on routine examines, immunizations and screenings.”

Patients are also encouraged to become active participants in their care, and their medical home will work to educate patients about healthy living and resources in the community that can further promote health and well-being.

In some ways, the family medicine model, formally recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home, re-establishes the traditional importance of the family doctor, but within the context of modern electronic medical record keeping and a highly complex medical care system.

Family practices that have earned NCQA Patient-Centered Medical Home Recognition provide their patients:

  • Short-notice appointments with a medical team member, often in the same day or within a couple days
  • Access to medical professionals through MyUCSDchart - a secure electronic portal, and by telephone, and
  • A primary care physician who can care for all family members.
