The Bariatric and Metabolic Institute offers a multidisciplinary team approach to weight loss. We believe that a strong support system is vital for successful weight loss and continuing good health.


Santiago Horgan 

Santiago Horgan, MD
Director, Bariatric and Metabolic Institute

Dr. Santiago Horgan is an internationally recognized expert in esophageal disease, bariatric surgery, and new technologies for minimally invasive approaches for surgery, which provide patients with a faster recovery and less scarring than open surgical methods. He performs a variety of weight-loss procedures, including gastric sleeve, gastric plication, gastric bypass, and revisional weight-loss surgeries, and was among the first surgeons to be trained in the use of the Lap-Band adjustable gastric banding system. Dr. Horgan is skilled in the use of surgical robotics including the da Vinci system, which enables surgeons to perform complex and delicate procedures through small incisions, and has trained other surgeons across the country on minimally invasive surgical techniques.  As the director of the Center for the Future of Surgery at UC San Diego School of Medicine, Dr. Horgan is working with colleagues to investigate, develop, test and teach procedures that are revolutionizing the field of surgery.

Eduardo Grunvald

Internist, Director of UC San Diego Weight Management Program

American Board of Medicine icon Dr. Grunvald is the internist for the UC San Diego Bariatric and Metabolic Institute and medical director of its Weight Management Program. He is integrally involved with managing patients undergoing surgical and nonsurgical weight loss efforts. Dr. Grunvald is responsible for assessing bariatric surgery patients before surgery and evaluating co-morbidities that may exist. Dr. Grunvald also coordinates care with primary care providers and assists patients with post-operative medication adjustments and medical follow-up.

Garth Jacobsen 

Garth Jacobsen, MD

Dr. Garth Jacobsen attended medical school at the University of Washington and received his surgical training at the University of Illinois, Chicago. He then completed a minimally invasive and bariatric surgery fellowship at UC San Diego. A specialist in gastric banding, he was closely involved with the FDA trials for the approval of the Lap-Band system.

William Perry, MD 

William Perry, PhD

Dr. Perry is the associate director of Neuropsychiatry and Behavioral Medicine and the chief supervising psychologist at UC San Diego Health. He is internationally recognized as an expert in assessment and has worked extensively in the psychological assessment of candidates for bariatric procedures. He is the senior member of the psychology team and an integral member of the Bariatric and Metabolic Institute.

Bryan Sandler, MD 

Bryan Sandler, MD

Dr. Bryan Sandler is fellowship-trained in minimally invasive and robotic surgery, and has additional specialized training in the surgical treatment of obesity. He is also active in the rapidly advancing field of natural orifice surgery. He treats patients who require general surgery for disorders of the foregut, esophagus, gastric or biliary system and small intestine. He also performs surgeries of the hindgut for colon and rectal disorders.


  • Mary Kirk, CNP - Nurse Manager
    Mary is a certified nurse practitioner. Her responsibilities at the Bariatric and Metabolic Institute include program planning, development and quality assurance, patient evaluations and follow-up (including diabetes management and medication adjustment), surgery-related presentations and assisting with general surgery.
  • Jill Fisher, MS, RD – Registered Dietitian
    Jill is the BMI dietitian and works with patients to help them through the pre-op and post-op phases of diet and meal planning, as well as providing tips for healthy eating to maximize patients' success.
  • Natalie Cook - Bariatric Patient Coordinator
    Natalie has been a patient counselor and care coordinator at the Bariatric and Metabolic Institute since 2007. She is well-versed in all bariatric surgical options and provides support and education throughout the entire process, from initial consultation to the recovery period following surgery. Natalie looks forward to helping you on your weight loss journey.
