Please have each student and faculty complete this agreement ONLINE prior to starting the assigned rotation and print the certificate. Information Services will assign computer access once received.
Confidentiality and Non-Disclosure Agreement
Please have each student and faculty complete the agreement and submit to the Nursing School Liaison.
Student/Faculty Document Checklist
Faculty may use this as a tracking tool to ensure all required documents are collected for each student. Submit the form with the required documents to Nursing School Liaison.
School coordinators fully complete SOR and email Student Orientation Records to Nursing School Liaison. UCSD can accept Student Orientation Records emailed with SS#. Schools can also password-protect document and send password in a separate email referencing consortium #.
Access the UC Learning Center under NON-UCSD Employees. Username is the date of birth and the last 4 numbers of the Social Security # (with no spaces) for a total of 12 digits (mm/dd/yyyy + last 4 of SS#). Students and Faculty must complete ALL courses assigned under the CURRENT REGISTRATION tab. PRINT certificate of completion and submit to Nursing School Liaison.
Parking Information for Nursing Students
For Current Employees
If you are a UCSD employee, please continue to park in the same way you have been on campus. This will not affect your current parking process.
For Student Nurses
When we onboard student nurses in our system, we provide you with an AD login (what you use to log into EPIC) that automatically signs you up for a UCSD Health email address. Your UCSD Health email address should be your AD login + @health.ucsd.edu (ex. johndoe@health.ucsd.edu)
To use our ParkMobile app at all locations with the discount codes, you must create an account in ParkMobile using the UCSD Health email address.
Before coming to Clinical:
- Download the ParkMobile app
- Register for a UCSD ParkMobile Account with your Health email address (not with your personal email address)
- Enter your car & payment information
When you arrive on-site:
- Park in the circled area (Maps attached )
- Click on the "PARK" icon to search for the parking zone; depending on where you park, enter & search the zone code to match, Click UCSD – Valid B, S, D Spaces.

- UCSD Bachman Parking Structure (Blue circle)
- "B" spot - $5.70/day. Enter (47300) into the Search Zones.
- Street along Bachman Place (Bachman Hill)
- Street spots - $4.25/day. Enter (4798) into the Search Zones and Enter your Space #.

- Athena Parking & Open Lot (across the street from Athena) (Blue Circle)
- Any "B" spot - $5.70/day. Enter (47300) into the Search Zones.
- Discount Lot (red circle)
- Unmarked spots (behind Athena Parking Garage) - $4.25/day. Enter 4762 into Search Zones

- Go to clinical following the Green Arrow on the Map provided.

Parking Tickets
During this transition period, if you encounter any issues with the ParkMobile app, we will take care of them. If ParkMobile has any issues, please do NOT use the ParkMobile account & please risk getting a parking ticket. If you get a ticket, please forward your ticket number or picture of the ticket to me (meyap@health.ucsd.edu) so that I can follow up with our parking service team to VOID.
